health care services. The services are made available to meet the
specific
needs of each patient. Our team of registered nurses is responsible for
developing an individualized plan of care and working with your primary
physician to insure appropriate care is rendered. Wound
Care is
one of Omega Home Health’s areas of special focus. All staff members of our care team are trained in
the prevention, evaluation, and treatment of skin wounds. The wound
management
program uses state-of -the- art techniques that can include:
By following
nationally recognized wound care standards and clinical paths, we are
able to
achieve quality outcomes. The wound care team is comprised of the patient,
care provider, personal physician, specially trained nurses, therapist,
medical
social worker, nutritionist, and home health aide. The program is
directly
supervised and monitored by Rhonda DeDear, RN. The wound care team develops a
comprehensive care plan to meet the patient’s needs, team member’s work
together
to promote healing and maintain healthy skin. Specially designed
protocols are
followed to effectively treat various wounds, including skin tears,
pressure
ulcers, venous leg ulcers, and diabetic foot ulcers Skilled Nursing Services are provided by Registered Nurses (RN's) and Licensed Vocational Nurses ( LVN's). Nurses provide the skilled care to meet a medical need and care is delivered in accordance with orders given by a physician and defined in a care plan. Registered Nurses coordinate care between the different disciplines by acting as the case manager. Home Health Aides carry out health care tasks under the supervision of a Registered Nurse. Home Health Aides provide assistance with personal hygiene, housekeeping and other related supportive tasks to a patient with health care needs in their home. Physical Therapists specialize in treatments that improve the mobility and physical strength of patients. Physical therapy primarily consists of physical exercise to alleviate pain and restore muscles along with patient teaching of special techniques to improve mobility. Occupational Therapists specialize in treatments that enable patients to perform the general activities of daily living (ADL's) such as bathing, eating, dressing and basic household routines. Generally individuals who have physical, developmental and/or social problems might need the skills of an Occupational Therapist. Speech Language Pathologists specialize in treatments that aid patients to restore speech that may have been lost due to some form of trauma, for example a stroke. Speech Therapists also train patients to improve their breathing, swallowing as well as their speech.
